Glendale Doctor Gets 15 Years for Medicare Botox Fraud
Violetta Mailyan, 45, who owned Healthy Way Medical Center in Glendale, was sentenced to 15 years in federal prison after a jury found her guilty of wire fraud on nine charges and of obstructing a health care fraud investigation on three. A jury found her guilty in May 2026 after a six-day trial.
Prosecutors said Mailyan billed Medicare for thousands of Botox treatments that were not performed, were cosmetic, or did not meet the program’s requirements for treatment of chronic migraines. Medicare covers the injections for documented chronic migraine cases when medically necessary.
More than $19 million in claims involved treatments supposedly given on days the clinic was closed. Prosecutors also said she billed for injections while away on trips to Hawaii, New York, Mexico, Pennsylvania and Las Vegas, including more than $200,000 in claims for California patients while she was in Cabo.
As investigators closed in, Mailyan falsified medical records and forged beneficiary signatures, prosecutors said. One forged signature was attributed to a man who was in federal prison when the records claimed he received treatment.
Prosecutors said she used the proceeds to buy luxury goods and property, including a $12,000 17th-century crossbow, a $3,000 painting, real estate and two Tesla vehicles, one of them a Cybertruck. The court ordered her to forfeit more than $16 million in assets and repay Medicare $32,904,049.41.
The sentencing judge described the fraud as extraordinary in scale and duration, and its execution as exceptionally brazen.
